Your rights
Access: You may inspect and request a copy of your PHI.
Amendment: You may request an amendment to PHI you believe is incorrect or incomplete.
Accounting of disclosures: You may request an accounting of certain disclosures of your PHI.
Restrictions: You may request restrictions on certain uses and disclosures. We are not required to agree, except as required by law.
Confidential communications: You may request that we communicate with you in a specific way or at a specific location.
Paper copy: You may request a paper copy of this notice at any time.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our practice or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
Changes to this notice
We reserve the right to change this notice. Changes apply to all PHI we maintain and will be posted on this website with the updated effective date.
